Objectifs : Évaluer la compréhension et les attentes des femmes en phase de déclenchement du travail, évaluer leur expérience réelle du processus et comparer leur satisfaction à l’égard du travail à celle des femmes qui ont un travail spontané.

Plan de l’étude : Quatre cent cinquante femmes à terme et nécessitant déclenchement du travail et de maturation cervicale avec des comprimés vaginaux de prostaglandine E2 et 450 femmes travaillant spontanément ont été recrutées pour cette étude. Le groupe de déclenchement a été prié de remplir un questionnaire avant le début du processus de déclenchement et un autre questionnaire après l’accouchement. Le questionnaire rempli après l’accouchement comprenait deux sections, l’une concernant les problèmes liés au déclenchement et l’autre au processus de travail proprement dit. Le groupe du travail spontané a été prié de remplir un questionnaire après l’accouchement, qui ne contenait que la section relative au processus de travail proprement dit. Les principaux critères de jugement étaient la satisfaction à l’égard du travail, la perception de la douleur et la durée du travail entre les groupes « travail déclenché » et « travail spontané », ainsi que les difficultés rencontrées que les femmes souhaiteraient voir améliorer dans le cadre du déclenchement.

Résultats : Dans le groupe du déclenchement, 34,7% des femmes n’étaient pas satisfaites des informations qu’elles avaient reçues sur le déclenchement avant la procédure et 27,2% s’attendaient à voir des résultats dans les 12 heures suivant l’administration de l’agent déclencheur. Après le déclenchement, 40% des femmes estimaient que l’aspect le plus important qu’elles souhaiteraient changer, si elles devaient à nouveau avoir recours à un déclenchement, serait la rapidité du déclenchement, 13,6% estimaient qu’elles souhaiteraient une prise orale de l’agent déclencheur, 7% souhaiteraient moins d’examens vaginaux et 9% moins de complications. Parmi les femmes qui ont rempli les questionnaires, 26,3% ont eu une césarienne dans le groupe du déclenchement et 21,4% dans le groupe d’accouchement spontané. Davantage de femmes étaient satisfaites de leur travail dans le groupe de travail spontané (79,5% contre 70,4%, RR 0,89, IC 95% 0,8–0,96, P = 0,006) par rapport à celui du travail déclenché.

Conclusions : Le travail provoqué artificiellement entraîne un taux de satisfaction des femmes inférieur par rapport au travail spontané. Le délai plus long entre le début du déclenchement et l’accouchement y joue un rôle important. Le mode d’administration de l’agent déclencheur, la multiplication des examens vaginaux et l’augmentation du nombre d’accouchements par césarienne sont perçus comme des problèmes secondaires. Il est nécessaire d’améliorer les informations fournies aux femmes en phase de déclenchement du travail afin de répondre aux attentes irréalistes et d’améliorer ainsi la satisfaction.

Abstract (English)  :

Objectives: To evaluate the understanding and expectations of women undergoing labour induction, to assess their actual experience of the process and to compare their satisfaction with labour to those labouring spontaneously.

Study design: Four hundred and fifty women at term undergoing induction of labour and cervical ripening with prostaglandin E2 vaginal tablets and 450 women labouring spontaneously were recruited into the study. The induction group were requested to complete a questionnaire prior to the start of their induction process and another questionnaire post-delivery. The post-delivery questionnaire contained two sections, one pertaining to issues to do with the induction and the second with the actual labour process. The spontaneously labouring group was requested to complete a questionnaire post-delivery, which only contained the section pertaining to the actual labour process. The main outcome measures were satisfaction with labour, perception of pain and length of labour between the induced and spontaneous labour groups, and issues that the women might wish changed about their induction.

Results: In the induction group, 34.7% were not satisfied with the information they received about the induction prior to the procedure and 27.2% expected to deliver within 12 h of the administration of the inducing agent. Post-induction, 40% of the women felt the most important aspect they would like to change about their induction were they to have another one, would be the speed of the induction, 13.6% felt they might wish to take the inducing agent orally, 7% to have fewer vaginal examinations and 9% to have fewer complications. Among the women who returned questionnaires, 26.3% had a caesarean delivery in the induction group and 21.4% in the spontaneous labour group. Significantly more women were satisfied with their labour in the spontaneous labour group 79.5% versus 70.4%, RR 0.89, 95% CI 0.8–0.96, P = 0.006).

Conclusions: Labour that is artificially induced does result in lower satisfaction rates as compared to that following spontaneous onset. The longer time delay between the start of the induction and the delivery plays a significant part in this, with the mode of administration of the inducing agent, more vaginal examinations and the increase in caesarean deliveries being perceived as secondary issues. There is a need to improve the information provided to women undergoing labour induction, to counter unrealistic expectations and thereby improve satisfaction.
