OBJECTIFS: Le but de cette etude etait d’evaluer l’impact du massage perineal realise pendant la grossesse sur les symptomes perineaux 3 mois apres l’accouchement.

STRUCTURE DE L’ETUDE: Des femmes enceintes de 5 hopitaux de la province du Quebec, Canada, ont participe a cette etude randomisee, controllee, en simple aveugle. Toutes les participantes ont recu une information orale et ecrite sur la prevention des traumatismes perineaux. La technique du massage perineal a ete expliquee aux femmes du groupe experimental, en leur demandant de realiser un massage quotidien de 10 minutes a partir de la 34ieme ou 35ieme semaine de grossesse jusqu’a l’accouchement. Les participantes ont complete elle-memes un questionnaire au moment de leur recrutement puis 3 mois apres l’accouchement sur la douleur perineale, la dyspareunie, la satisfaction sexuelle, ainsi que l’incontinence urinaire, au gaz, et fecale.

RESULTATS: Parmi les primipares il n’y avait pas de differences entre le groupe massage (n=283) et le groupe de controle (n=289) en ce qui concerne la douleur perineale, la dyspareunie, la satisfaction sexuelle, et les incontinences, a 3 mois post-partum. Parmi les multipares les femmes du groupe massage (n=187) avaient moins de douleur perineale que celles du groupe de controle (n=190), mais la frequence des dyspareunies et incontinences etaient similaires dans les 2 groupes.

CONCLUSIONS: Le massage perineal pendant la grossesse n’abime ni ne protege la fonction perineale 3 mois apres l’accouchement.

Abstract (English)  :

OBJECTIVE: The aim of this study was to evaluate the effect of perineal massage performed during pregnancy on perineal symptoms 3 months after delivery.

STUDY DESIGN: Pregnant women from 5 hospitals in the province of Quebec, Canada, participated in this single-blind, randomized, controlled trial. All participants received oral and written information on the prevention of perineal trauma. Women in the experimental group were taught the perineal massage technique and were asked to perform a 10-minute perineal massage daily from the 34th through 35th weeks of pregnancy until delivery. Participants completed a self-administered questionnaire on perineal pain, dyspareunia, sexual satisfaction, and incontinence of urine, flatus, and stool at the time of enrollment and 3 months after delivery.

RESULTS: Among participants without a previous vaginal birth there were no differences between the massage (n = 283) and the control (n = 289) groups with respect to perineal pain, dyspareunia, sexual satisfaction, and incontinence of urine, gas, or stool 3 months post partum. Among women with a previous vaginal birth more women in the massage group (n = 187) than in the control group (n = 190) were free of perineal pain (93.6% vs 85.8%; P =.01) but the frequencies of dyspareunia and incontinence of urine, gas, or stool were similar in the 2 groups.

CONCLUSIONS: Perineal massage during pregnancy neither impairs nor substantially protects perineal function at 3 months post partum.

Lorsque l’on comptabilise les dechirures visibles, le massage perineal pendant la grossesse semble proteger le perinee, au moins pour les primipares. Mais lorsque l’on considere la douleur, la dyspareunie, et les incontinences, a 3 mois post-partum, ce massage n’a aucun effet.
