Nous avons étudie l’état de stress post-traumatique après l’accouchement en fonction des paramètres obstétricaux sur un échantillon de 1550 récentes accouchées.
Le profil symptomatique de l’état de stress post-traumatique (ESPT) et les symptômes de stress traumatique ont été évalués en utilisant l’échelle des évènements traumatiques (EET). Les paramètres obstétricaux incluaient le mode d’accouchement, la durée de la seconde phase de travail (d’une dilatation cervicale de 10 cm à la naissance), et l’utilisation d’analgésie/anesthésie. Les symptômes de stress traumatique, ayant de plus un profil symptomatique d’ESPT étaient tous deux significativement corrélés aux césariennes en urgence ou aux accouchements vaginaux instrumentaux. Sur le plan clinique, il est cependant important de noter que la majorité des femmes avec un profil symptomatique d’ESPT étaient dans le groupe des accouchements vaginaux normaux. Cela signifie qu’un accouchement vaginal normal peut être ressenti comme traumatique, de la même façon qu’une césarienne en urgence n’est pas nécessairement traumatique. Les symptômes de stress traumatique n’étaient pas significativement corrélés à la durée de la seconde phase de travail, ni à l’utilisation d’analgésie/anesthésie.

Abstract (English)  :

In a sample of 1550 recently delivered women, traumatic stress after childbirth was studied in relation to obstetric variables.
A post-traumatic stress disorder (PTSD) symptom profile and traumatic stress symptoms were assessed by means of the Traumatic Event Scale (TES). Obstetric data comprised delivery mode, duration of the second stage of labor (the time from cervical dilation of 10 cm to partus) and the use of analgesia/anesthesia.
Traumatic stress symptoms and having a PTSD symptom profile were both significantly related to the experience of an emergency cesarean section or an instrumental vaginal delivery. It is of clinical importance, however, that most women with a PTSD symptom profile were found in the normal vaginal delivery group (NVD). This implies that a normal vaginal delivery can be experienced as traumatic, just as an emergency cesarian section is not necessarily traumatic. Traumatic stress symptoms were neither substantially correlated to the duration of the second stage of labor, nor to the use of analgesia/anesthesia.
