Résumé (français) : | Cette étude examine le risque de malformations congénitales structurelles spécifiques associées à l’exposition à l’ondansétron au cours du premier trimestre chez une large population américaine assurée sur le plan commercial. Les données relatives aux réclamations médicales ont été obtenues auprès de Truven Health Analytics pour 864 083 paires mère-enfant de 2000 à 2014. Une régression logistique a été utilisée pour mesurer l’association entre l’exposition à l’ondansétron pendant le premier trimestre et le risque d’anomalies cardiaques, de fentes orofaciales et d’autres défauts structurels spécifiques chez les enfants. L’exposition à l’ondansétron pendant le premier trimestre a été associée à un risque accru de défauts cardiaques cardiaques (OR : 1,52 à 95% : 1,35-1,70) et fente orofaciale (OR : 1,32 à 95% : 0,76-2,28) par rapport aux femmes sans exposition antiémétique au cours de la grossesse. Cette analyse aborde les limites des études antérieures, notamment en ce qui concerne la consommation d’énergie, les erreurs de classification des expositions et la généralisation à la population américaine. Dans une grande population américaine, nous avons trouvé une association statistiquement significative entre l’exposition à l’ondansétron au début de la grossesse et des anomalies congénitales structurelles spécifiques chez la progéniture. |
Abstract (English) : | This study investigates risk of specific structural birth defects associated with ondansetron exposure during the first trimester in a large US commercially-insured population. Medical claims data were obtained from Truven Health Analytics for 864,083 mother-infant pairs from 2000 to 2014. Logistic regression was used to measure the association between first trimester exposure to ondansetron and risk of cardiac defects, orofacial clefts and other specific structural defects in offspring. First trimester exposure to ondansetron was associated with increased risk of cardiac (OR: 1.52 95% CI: 1.35-1.70) and orofacial cleft defects (OR: 1.32 95% CI: 0.76-2.28) in offspring compared to women with no antiemetic exposure during pregnancy. Os dados de reclamações médicas foram obtidos no Truven Health Analytics para 864.083 pares mãe-bebê de 2000 a 2014. A regressão logística foi usada para medir a associação entre a exposição do primeiro trimestre ao ondansetron e o risco de defeitos cardíacos, fissuras orofaciais e outros defeitos estruturais específicos da prole.
