À l’heure actuelle, les résultats périnataux en France demeurent parmi les moins satisfaisants en Europe malgré une très forte médicalisation. Les autorités de santé affirment reconnaître l’influence de la prise en charge et de l’accompagnement pendant la grossesse, l’accouchement et la période néonatale sur l’état de santé de la mère et de l’enfant mais l’offre de soins peine à s’élargir. Contrairement à ce que prévoient les lois concernant le libre choix du praticien en France, les parturientes ne sont pas à même de choisir le professionnel de santé qui les accompagnera lors de leur accouchement. De même, les sages-femmes, contrairement à ce qu’impose leur code de déontologie, ne sont pas en mesure de garantir à leurs patientes ni les conditions, ni le lieu de leur accouchement. De surcroît, peu d’entre elles exercent leur profession de façon autonome tout en pratiquant le plein exercice de leur fonction.

En centrant cette recherche anthropologique sur l’expérience des femmes et des sages-femmes dans leurs relations autour de la naissance et de l’accouchement, l’objectif de cette étude qualitative est de faire la lumière sur les tenants et les aboutissants de l’uniformisation de l’offre de soins. Dans un premier temps sont abordés les différents types d’accompagnement proposés aux femmes aujourd’hui ainsi que la place qu’y occupent les sages-femmes. Dans un second temps, l’étude de l’accompagnement global permet l’examen de la prise en charge des patientes et de ses liens de causalité avec les relations interprofessionnelles. Ainsi sont révélés les impacts de cette pratique sur le soutien à la parentalité et la nécessité d’une diversification de l’offre de soins.

Abstract (English)  :

At present, perinatal results in France remain among the least satisfactory in Europe despite very strong medicalization. Health authorities claim to recognize the influence of care and support during pregnancy, childbirth and the neonatal period on the state of health of the mother and the child but the offer of care hardly expands. Contrary to what is stipulated by the laws concerning the free choice of a practitioner in France, parturients are not able to choose the health professional who will accompany them during their delivery. Similarly, midwives, contrary to their code of ethics, are unable to guarantee their patients either the conditions or the place of delivery. Moreover, few of them practice their profession autonomously in full exercise.

By focusing this anthropological research on the experience of women and midwives in their relations around birth and childbirth, the objective of this qualitative study is to shed light on the outcomes of standardizing the offer of care. Initially, the different types of accompaniment offered to women are discussed, as well as the place occupied by midwives. In a second step, the study of the global support allows the examination of the patients’ care and its links of causality with the interprofessional relations. Thus, the impacts of this practice on support for parenthood and the need for a diversification of the supply of care are revealed.

Atualmente, os resultados perinatais na França permanecem entre os menos satisfatórios da Europa, apesar da forte medicalização. As autoridades de saúde dizem reconhecer a influência do cuidado e apoio durante a gravidez, parto e período neonatal em cuidados de saúde e da criança da mãe, mas oferta dificilmente expandir. Ao contrário do que fornecer as leis relativas à livre escolha do médico na França, as mulheres em trabalho de parto não são capazes de escolher o profissional de saúde que irá acompanhá-los durante o parto. Da mesma forma, parteiras, ao contrário do que é exigido por seu código de conduta, não são capazes de proporcionar aos seus pacientes nem as condições, nem o local de seu nascimento. Além disso, poucos praticam sua profissão de forma autônoma enquanto praticam o pleno exercício de sua função.
Centrando esta pesquisa antropológica sobre as experiências das mulheres e parteiras nas suas relações em torno de nascimento e nascimento, o objetivo deste estudo qualitativo é lançar luz sobre os ins e os resultados da padronização da oferta de cuidados. Inicialmente, são discutidos os diferentes tipos de acompanhamento oferecidos às mulheres, bem como o lugar ocupado pelas parteiras. Em segundo lugar, o estudo de suporte global permite o exame da gestão dos pacientes e suas relações causais com relações interprofissionais. Assim, revelam-se os impactos dessa prática no apoio à parentalidade e a necessidade de diversificação da oferta de cuidados.
