Le but de cette étude prospective était d’examiner l’étiologie des symptômes de stress post-traumatique après l’accouchement dans un cadre transactionnel de stress. Les participants étaient des femmes (N = 1 499) de la cohorte de naissance d’Akershus. Ces femmes ont été suivies de la grossesse à 8 semaines après l’accouchement. Nous avons modélisé les facteurs prédisposants (p. Ex., La peur de l’accouchement) et les facteurs déclenchants (expériences de naissance subjectives et objectives) en tant que prédicteurs des symptômes de stress post-traumatique. Les symptômes de stress post-traumatique ont été mesurés au moyen de l’impact de l’échelle des événements, d’expériences de naissance objectives au moyen de journaux de naissance et d’expériences de naissance subjectives au moyen de trois questions. Un modèle d’équation structurelle a montré que les expériences de naissance subjectives étaient les plus associées aux symptômes de stress post-traumatique. De plus, ils ont influencé l’effet des facteurs prédisposants et des expériences de naissance objectives. Les résultats suggèrent que les expériences de naissance subjectives des femmes sont le facteur le plus important dans le développement des symptômes de stress post-traumatique après l’accouchement.

Abstract (English)  :

The aim of this prospective study was to examine the etiology of post-traumatic stress symptoms following childbirth within a transactional framework of stress. Participants were women (N = 1,499) from the Akershus Birth Cohort. These women were followed from pregnancy to 8 weeks postpartum. We modeled predisposing factors (e.g., fear of childbirth) and precipitating factors (subjective and objective birth experiences) as predictors of post-traumatic stress symptoms. Post-traumatic stress symptoms were measured by means of the Impact of Event Scale, objective birth experiences by means of birth journals, and subjective birth experiences by means of three questions. A structural equation model showed that subjective birth experiences had the highest association with post-traumatic stress symptoms. Moreover, they mediated the effect of predisposing factors and objective birth experiences. The results suggest that women’s subjective birth experiences are the most important factor in the development of post-traumatic stress symptoms following childbirth.

O objetivo deste estudo prospectivo foi examinar a etiologia dos sintomas de estresse pós-traumático após o parto dentro de um quadro transacional de estresse. Os participantes eram mulheres (N = 1.499) da Coorte de Nascimentos de Akershus. Estas mulheres foram acompanhadas desde a gravidez até 8 semanas após o parto. Nós modelamos fatores predisponentes (por exemplo, medo do parto) e fatores precipitantes (experiências de nascimento subjetivas e objetivas) como preditores de sintomas de estresse pós-traumático. Os sintomas de estresse pós-traumático foram medidos por meio da Escala de Impacto do Evento, das experiências objetivas de nascimento por meio de publicações de nascimento e das experiências subjetivas de nascimento por meio de três perguntas. Um modelo de equação estrutural mostrou que as experiências subjetivas de nascimento tiveram a maior associação com sintomas de estresse pós-traumático. This study used structural equation modelling to examine associations between qualitative categories (subjective birth experiences) and objective facts about the births of 1499 women. This is an interesting mixed-methods approach to the problem. In essence it supports the need for continuous care, compassion and a sense of control for all women during the intrapartum period to prevent post-natal traumatic stress.

Cette étude a utilisé la modélisation par équation structurelle pour examiner les associations entre les catégories qualitatives (expériences subjectives de naissance) et les faits objectifs sur les naissances de 1499 femmes. C’est une approche intéressante des méthodes mixtes au problème. Essentiellement, il soutient le besoin de soins continus, de compassion et de sens de contrôle pour toutes les femmes pendant la période intrapartum afin de prévenir le stress post-natal.

Este estudo usou a modelagem de equações estruturais para examinar associações entre categorias qualitativas (experiências subjetivas de nascimento) e fatos objetivos sobre os nascimentos de 1499 mulheres. Esta é uma abordagem interessante de métodos mistos para o problema. Em essência, ele apóia a necessidade de cuidados contínuos, compaixão e um senso de controle para todas as mulheres durante o período intraparto para evitar o estresse pós-natal traumático.
