Les progrès au début du travail sont généralement lents et peuvent inclure des contractions utérines douloureuses. Les femmes peuvent se sentir en détresse et perdre confiance en elles au cours de cette phase. Les interventions de soutien et d’évaluation ont été évaluées dans deux revues Cochrane précédentes. Cet avis met à jour et remplace ces deux avis.

L’évaluation et le soutien au début du travail n’ont pas d’effet évident sur le taux de césarienne, de naissance à l’instrument ou de naissance avant l’arrivée à l’hôpital. Cependant, certaines preuves suggèrent que les interventions pourraient avoir un impact sur la réduction de l’utilisation de l’anésthésie péridurale et sur l’augmentation de la satisfaction maternelle vis-à-vis des soins. Les preuves sur l’utilisation de l’ocytocine pour l’augmentation du travail étaient mitigées. Les preuves de l’efficacité de l’évaluation précoce du travail par rapport à l’admission immédiate étaient très limitées et des recherches supplémentaires sont nécessaires dans ce domaine.

Abstract (English)  :

Progress in early labour is usually slow and may include painful uterine contractions. Women may feel distressed and lose their confidence during this phase. Support and assessment interventions have been assessed in two previous Cochrane Reviews. This review updates and replaces these two reviews.

Assessment and support in early labour does not have a clear impact on rate of caesarean section or instrumental birth, or birth before arrival at hospital. However, some evidence suggested that interventions may have an impact on reducing the use of epidural, and on increasing maternal satisfaction with care. Evidence on the use of oxytocin for labour augmentation was mixed. Evidence about the effectiveness of early labour assessment versus immediate admission was very limited and more research is needed in this area.
