L’étude de la littérature scientifique des partisans et opposants à l’épisiotomie montre que cette pratique devrait être utilisée avec parcimonie. Bien que l’on admette que l’épisiotomie protège des déchirures périnéales antérieures, il n’y a aucune donnée factuelle à l’appui de la croyance qu’elle protège des déchirures du sphincter anal, des lésions du muscle pelvien, de l’incontinence urinaire de la parturiente, ni de l’hémorragie intercrânienne ou de l’asphyxie intrapartum chez le nouveau-né.

Les femmes qui subissent des épisiotomies perdent plus de sang pendant l’accouchement, et l’épisiotomie comporte un risque de mauvaise cicatrisation et d’augmentation de la douleur postpartum à court terme. La pratique de l’épisiotomie a régressé en Suède au cours des 20 dernières années, pour se situer aujourd’hui [1998] à environ 5% pour les multipares et 15% pour les primipares.

Abstract (English)  :

A review of the literature on the pros and cons of episiotomy suggests that it should be used restrictively. Although there is a general agreement that episiotomy is protective against anterior perineal tears, no evidence has been found to support the belief that it is protective against anal sphincter muscle tears, pelvic muscle damage or urinary incontinence in the parturient or intracranial haemorrhage or intrapartum asphyxia in the newborn.

Women undergoing episiotomy are characterised by greater blood loss in conjunction with delivery, and there is a risk of improper wound healing and increased pain during the early puerperium. The prevalence of episiotomy in Sweden has been declining over the past 20 years, and is now approximately five per cent among multiparae and fifteen per cent among primiparae.
