Résumé (français) : | Le microbiote vaginal, dominé par Lactobacillus crispatus, L. jensenii et L. gasseri, protège la muqueuse contre l’établissement de micro-organismes pathogènes par trois mécanismes complémentaires : a) l’adhésion spécifique à l’épithélium, qui bloque leur installation, b) la production de composés antimicrobiens et c) la co-agrégation avec les pathogènes, qui renforce leur effet microbiocide. Malgré cela, elle est parfois remplacée par des micro-organismes indésirables, ce qui est associé au développement d’une vaginose bactérienne, de la vaginite à Candida spp., de la trichomonase et des infections du bas appareil urinaire. Très rarement, les lactobacilles provoquent des pathologies, invariablement chez les patients immunodéprimés. Les cas prédominants sont la bactériémie (environ 50% des cas) et l’endocardite (30%). Cependant, aucune pathologie génitale due aux lactobacilles n’a été décrite. L’effet mutualiste des lactobacilles suggère que leur instillation pourrait régénérer l’écosystème vaginal, éliminant ainsi les récidives liées au traitement de l’infection. |
Abstract (English) : | Vaginal microbiota, mainly comprised of Lactobacillus crispatus, L. jensenii and L. gasseri, protect the mucosa against the establishment of pathogenic microorganisms through three complementary mechanisms: a) specific adherence to the epithelium, which blocks colonization of pathogens, b) production of antimicrobial compounds, and c) co-aggregation with pathogens, which enhances their microbiocidal effect. Despite these mechanisms, vaginal microbiota are sometimes displaced by undesirable microorganisms, which is associated with the development of bacterial vaginosis, vaginitis due to Candida spp., trichomoniasis, and lower urinary tract infections. On rare occasions, lactobacilli cause disease, but exclusively in immunocompromised patients. The main conditions are bacteremia (about 50% of the cases) and endocarditis (30%). However, no genital pathology caused by lactobacilli has been reported. The mutualistic effect of lactobacilli suggests that instillation of these microorganisms might regenerate the vaginal ecosystem, thus eliminating the relapses associated with treatment of the infection. |
Sumário (português) : | A microbiota vaginal, dominada por Lactobacillus crispatus, L. jensenii e L. gasseri, protege a mucosa contra o estabelecimento de microrganismos patogênicos por três mecanismos complementares: a) adesão específica ao epitélio, que bloqueia sua instalação, b) produção de compostos antimicrobianos ec) co-agregação com patógenos, o que aumenta seu efeito microbiocida. Apesar disso, às vezes é substituído por microrganismos indesejáveis, que estão associados ao desenvolvimento de vaginose bacteriana, Candida spp., Vaginite, tricomoníase e infecções do trato urinário inferior. Muito raramente, os lactobacilos causam patologias, invariavelmente em pacientes imunocomprometidos. Os casos predominantes são bacteremia (cerca de 50% dos casos) e endocardite (30%). No entanto, nenhuma patologia genital devido a lactobacilos foi descrita. O efeito mutualístico dos lactobacilos sugere que sua instilação poderia regenerar o ecossistema vaginal, eliminando assim as recorrências associadas ao tratamento da infecção. |
