OBJECTIF :
Cet article cherche à déterminer si les comportements interactifs dyadiques ont été influencés par le stress et les sentiments des parents dans les dyades mère-enfant prématurées et à terme.
METHODES :
45 mères (âge = 35,29 ± 5,38) et pères (âge = 36,77 ± 6,89) de nouveau-nés prématurés (GA = 30,25 ± 2,95 ; PC = 1288,02 ± 488,76) et 36 mères (32,60 ± 4,56) et pères (âge = 35,54 ± 5,16) à terme complet (GA = 39,88 ± 1,38 ; PC = 3156,39 ± 493,81) ont été impliqués. Les parents ont rempli l’échelle d’impact de l’événement révisée (IES-R), le profil des états d’humeur (POMS) et la forme abrégée de l’indice de stress parental (PSI-SF) et les comportements interactifs (Échelle d’évaluation globale) ont été filmés au bout de 3 mois.
RÉSULTATS :
Les mères d’enfants prématurés présentaient un niveau plus élevé d’intrusion (Mpreterm = 4,07 ± 0,74, Mfullterm = 4,39 ± 0,51, t = 2,22, p = 0,029) et d’éloignement (Mpreterm = 4,45 ± 0,83, Mfullterm = 4,79 ± 0,34, t = 2,51, p = 0,015) que les mères d’enfants nés à terme. Dans les niveaux inférieurs de sensibilité des mères prématurées, des niveaux plus élevés d’intrusion, d’éloignement et de dépression sont associés à la présence de sentiments négatifs et de stress parental chez les deux parents. De plus, la détresse chez les enfants plus élevés est associée aux sentiments négatifs des parents, au stress paternel et aux symptômes post-traumatiques. Un score plus élevé de sentiments négatifs des parents et de stress parental prédit des scores plus faibles dans les dimensions de Global RatingScale.
CONCLUSIONS :
Nos résultats soulignent que la prématurité pourrait être un facteur de risque pour la co-construction d’échanges interactifs entre mère et bébé prématuré. Cette étude pourrait aider les praticiens à mieux prendre en compte les rôles parentaux et à mener des interventions de soutien spécifiques pour les parents et les enfants.

Abstract (English)  :

AIM:
This paper aims to investigate if the dyadic interactive behaviours were influenced by parental stress and feelings both in preterm and full-term mother-child dyads.
METHODS:
45 mothers (age = 35.29 ± 5.38) and fathers (age = 36.77 ± 6.89) of preterm infants (GA = 30.25 ± 2.95; BW = 1288.02 ± 488.76), and 36 mothers (age = 32.60 ± 4.56) and fathers (age = 35.54 ± 5.16) of full-term (GA = 39.88 ± 1.38; BW = 3156.39 ± 493.81) were involved. Parents filled out the Impact of Event Scale Revised (IES-R), Profile of Mood States (POMS) and Parenting Stress Index Short Form (PSI-SF) and interactive behaviours (Global Rating Scale) was videotaped after 3 months.
RESULTS:
Mothers of preterm children showed higher level of Intrusiveness (Mpreterm = 4.07 ±.74, Mfullterm = 4.39 ±.51, t = 2.22, p =.029) and Remoteness (Mpreterm = 4.45 ±.83, Mfullterm = 4.79 ±.34, t = 2.51, p =.015) than mothers of term children. In preterm mothers’ lower levels of Sensitivity, higher levels of Intrusiveness, Remoteness and Depression are associated with the presence of negative feelings and parental stress in both parents. Moreover, higher children Distress is associated to parental negative feelings, paternal stress and post-traumatic symptoms. A higher score of parental negative feelings and parental stress predicted lower scores in Global RatingScale dimensions.
CONCLUSIONS:
Our results underline that preterm birth could be a risk factor for the co-construction of interactive exchanges between mother and premature baby. This study could help practitioners to better consider parental roles and to carry out specific supportive interventions for both parents and children.

OBJETIVO:
Este artigo investiga se os comportamentos interativos diádicos foram influenciados pelo estresse e pelos sentimentos dos pais nas díades prematuras e a termo mãe-filho.
MÉTODOS:
45 mães (idade = 35,29 ± 5,38) e pais (idade = 36,77 ± 6,89) de recém-nascidos prematuros (IG = 30,25 ± 2,95, PC = 1288,02 ± 488, 76) e 36 mães (32,60 ± 4,56) e pais (idade = 35,54 ± 5,16) a termo (IG = 39,88 ± 1,38, PC = 3156,39 ± 493,81) estavam envolvidos. Os pais completaram a Escala Revisada de Impacto de Eventos (IES-R), Perfil de Estado de Humor (POMS) e Formulário Curto do Índice de Estresse Parental (PSI-SF) e comportamentos interativos (Global Assessment Scale) foram filmados após 3 meses.
RESULTADOS:
Mães de prematuros apresentaram maior nível de intrusão (Mpreterm = 4,07 ± 0,74, Mfullterm = 4,39 ± 0,51, t = 2,22, p = 0,029) e afastamento (Mpreterm = 4,45 ± 0,83, Mfullterm = 4,79 ± 0,34, t = 2,51, p = 0,015) do que as mães de bebês a termo. Em níveis mais baixos de sensibilidade de mães prematuras, níveis mais altos de intrusão, afastamento e depressão estão associados à presença de sentimentos negativos e estresse dos pais em ambos os pais. Além disso, o sofrimento entre os filhos mais altos está associado a parentalidade negativa, estresse paterno e sintomas pós-traumáticos. Uma pontuação mais alta de sentimentos negativos dos pais e estresse dos pais previu pontuações mais baixas nas dimensões Global RatingScale.
CONCLUSÕES:
Nossos achados enfatizam que a prematuridade pode ser um fator de risco para a co-construção de trocas interativas entre mãe e bebê prematuro. Este estudo pode ajudar os profissionais a considerar melhor os papéis dos pais e fornecer intervenções de apoio específicas para pais e filhos.
