Objectifs : Aujourd’hui environ 200 millions de femmes ou fillettes ont subi des mutilations sexuelles dans le monde. 3 millions d’autres en sont menacées chaque année. Vieilles de plusieurs millénaires, les mutilations sexuelles féminines (MSF) sévissent sur tout le globe, principalement en Afrique. L’Europe et la France n’en dérogent pas puisque 42 000 à 61 000 personnes seraient touchées sur le territoire national. Les sages-femmes peuvent être amenées à côtoyer ces femmes et agir pour les accompagner. Dans un contexte où la prise en charge médicale conventionnelle se confronte à des spécificités traditionnelles, quelles sont les méthodes utilisées et les limites rencontrées par les sages-femmes pour évoquer les mutilations sexuelles en consultation ? Sur quels outils et ressources s’appuient-elles ? Quel est le retentissement de la formation sur cet échange ? Quelles représentations des mutilations se font les sages-femmes ?

Matériel et méthodes : Nous avons donc réalisé une étude qualitative basée sur des entretiens semi-directifs avec 12 sages-femmes ayant une pratique courante en consultation. Ont été incluses des sages-femmes hospitalières, libérales, territoriales et à activité mixte. Elles exerçaient au sein de structures accueillant une part importante de populations provenant des régions du monde les plus touchées par les mutilations sexuelles.

Résultats : Du point de vue technique, les sages-femmes interrogées utilisaient par défaut la communication verbale pour évoquer les (MSF), peu avaient recours à des supports iconographiques ou accessoires comme le miroir. Elles exprimaient quasi unanimement une volonté de prendre en charge, d’accompagner les patientes et de prévenir les MSF. Cependant, malgré l’expérience, elles pouvaient être limitées par une formation insuffisante ou peu ciblée sur les enjeux globaux des MSF ainsi que sur la mise en place d’un plan d’accompagnement pluridisciplinaire. Elles soulignaient également l’importance à accorder à la levée des tabous ainsi qu’à l’éducation à la santé des patientes pour une meilleure connaissance et maîtrise de leur corps.

Abstract (English)  :

Objective: Today, around 200 million of women and young girls have experienced genital mutilations in the world. In addition, other three million of individuals are threatened by this practice every year. Practiced during the last thousand years, genital mutilations are today affecting a significant portion of the world population, particularly the African population. Europe and France are not exempt from this practice as approximately 42,000 to 61,000 people are affected on the national territory. The midwives can be trained to get to know the victims and assist them to recover from that experience. In a context where the medical coverage is relative to specific customs, what are the methods used and the potential limits encountered by the midwives to approach the sexual mutilations during consultation? What are the resources and materials they can rely on? What is the impact of the training course to be expected from this exchange? Which representations of the sexual mutilations do the midwives adopt?

Methods: We have realized a qualitative study based on semi-directive interviews with 12 midwives who have a current practice in consultation. This study also includes hospital, liberal and Maternal-child Health Policy midwives with different activities. They exercised in the structures welcoming a significant part of populations coming from the most affected area in the world by the sexual mutilation practices.

Results and conclusion: In a technical approach, the midwives who were interrogated on that subject were using, on default, verbal communication as a way to approach the women of that phenomenon. Indeed, some had an option to rely on iconographic supports or accessories such as mirrors. They express their will to assist and take care of the female patients and to prevent such practices in the future. However, regardless of their experience, they could be limited by the insufficient amount of training course and the fact that the trainings are not focused on the consequences caused by the female genital mutilations on a global scale. In addition, the lack of multi-disciplinary assistance programs can prevent them from achieving their initial objectives which are to mitigate the old and bad practices of mutilation. The midwives also emphasized the importance of resolving the issues of taboos regarding that subject and to encourage the health education of patients for a better knowledge and master of their bodies.
