“Nullified“ (réduite à néant) : perception de femmes des maltraitances liées aux soins

Objectifs
Dans une étude effectuée à l’aide du questionnaire NorAQ (Norvold abuse questionnaire) auprès des patientes d’un centre de gynécologie, la prévalence des maltraitances dans le systèmes de soins (abuse in healthcare, AHC) allait de 13% à 28%. Pour notre étude, nous avons choisi une approche qualitative. Notre objectif était de comprendre en profondeur les maltraitances liées aux soins auxquelles les patientes (femmes) suédoises sont confrontées.

Conception de l’étude
Entretiens qualitatifs avec 10 patientes de centres de gynécologie qui ont été confrontées aux maltraitances liées aux soins. Les entretiens ont été analysés par “Grounded Theory“ (théorie à base empirique)

Résultats
La saturation [?] a été atteinte au bout de 6 entretiens. Les analyses font ressortir quatre catégories qui expliquent ce que les maltraitances liées aux soins signifient pour les participantes : elles se sont senties impuissantes (powerless), elles se sont senties ignorées, elles se sont senties négligées, et ont ressenti un manque de bienveillance. Ces quatre catégories peuvent être regroupées de façon théorique dans une catégorie primaire (core categorie) “to be nullified“ (être réduite à néant, être anéantie) Les récits des femmes décrivent une souffrance persistante intense, même lorsque les maltraitances ont eu lieu des années auparavant.

Conclusions
L’existence même des maltraitances liées aux soins est un dilemme pour une institution qui est chargée par la société de soigner ou de soulager la douleur et la souffrance. Dans leurs récits, les femmes décrivent avoir été “nullified“, une catégorie primaire qui concrétise les maltraitances liées aux soins

Abstract (English)  :

Objective
In a study performed with The NorVold Abuse Questionnaire (NorAQ) among Nordic gynecological patients, the prevalence of lifetime abuse in health care (AHC) was 13 - 28%. In the present study we chose a qualitative approach. Our aim was to develop a more in-depth understanding of AHC, as experienced by female Swedish patients.

Study design
Qualitative interviews with 10 Swedish gynecological patients who had experienced AHC. The interviews were analyzed through Grounded Theory.

Results
Saturation was reached after six interviews. In the analyses four categories emerged which explain what AHC meant to the participating women: felt powerless, felt ignored, experienced carelessness, and experienced non-empathy. To be nullified is the core category that theoretically binds the four categories together. The women’s narratives described intensive current suffering even though the abusive event had taken place several years ago.

Conclusions. The fact that AHC exists is a critical dilemma for an institution that has the society’s commission to cure and/or to alleviate pain and suffering. In their narratives, women described the experience of ’being nullified’, a core category that embodies AHC
