Résumé (français) : | Cette étude avait pour objet de comparer les risques d’hémorragie postpartum pour les femmes qui choisissent d’accoucher à domicile et celles qui choisissent d’accoucher à l’hôpital. Il s’agit d’une étude observationnelle basée sur une analyse des rapports de maternité à l’aide d’une modélisation en régression logistique binaire. Les données concernent les grossesses suivies par un parmi quinze hôpitaux de l’Autorite de santé du nord-ouest de la Tamise (North West Thames Regional Health Authority Area) en Angleterre, et qui ont abouti à une naissance vivante ou une mortinaissance dans les années 1988 à 2000 incluses, en excluant les grossesses “à haut risque“, les accouchements non planifiés à domicile, les naissances avant terme, les césariennes planifiées et le déclenchement médical. |
Abstract (English) : | The aim of this study is to compare the odds of postpartum haemorrhage among women who opt for home birth against the odds of postpartum haemorrhage for those who plan a hospital birth. It is an observational study involving secondary analysis of maternity records, using binary logistic regression modelling. The data relate to pregnancies that received maternity care from one of fifteen hospitals in the former North West Thames Regional Health Authority Area in England, and which resulted in a live or stillbirth in the years 1988-2000 inclusive, excluding ’high-risk’ pregnancies, unplanned home births, pre-term births, elective Caesareans and medical inductions. |

Argument (français) : | Le risque d’hémorragie postpartum est 2.5 fois plus élevé pour les accouchements planifiés à l’hôpital que pour ceux prévus à domicile. D’autres recherches seraient nécessaires pour savoir (a) si l’on retrouve le même schéma pour les formes d’hémorragie postpartum mettant en danger la vie de la parturiente, et (b) pourquoi l’accouchement à l’hôpital est associé à un risque accru d’hémorragie postpartum. |
Argument (English): | The risk of PPH is 2.5 times higher among births planned to take place in hospital compared to births planned to take place at home. Further research is needed to understand (a) whether the same pattern applies to the more life-threatening categories of PPH, and (b) why hospital birth is associated with increased odds of PPH. |
