Résumé (français) : | Cet article apporte un complément aux autres articles sur la maieutique (midwifery), en documentant les expériences des pays à faible et moyen revenu qui ont placé les sages-femmes au coeur de leur stratégie d’amélioration de la santé maternelle et du nouveau-né. L’article examine diverses interventions destinées à renforcer le système de santé au Burkina Faso, au Cambodge, en Indonésie et au Maroc. Le développement de la formation initiale des sages-femmes n’en était qu’une des constituante. Les efforts de renforcement du système de santé dans ces pays ont été caractérisés par: l’expansion du réseau d’établissements de santé avec une utilisation accrue des maternités, la formation de plus de sages-femmes, la réduction des obstacles financiers et une attention plus tardive à l’amélioration de la qualité des soins. Abstract (English) : | This paper complements the other papers in the Lancet Series on midwifery by documenting the experience of low-income and middle-income countries that deployed midwives as one of the core constituents of their strategy to improve maternal and newborn health. It examines the constellation of various diverse health-system strengthening interventions deployed by Burkina Faso, Cambodia, Indonesia, and Morocco, among which the scaling up of the pre-service education of midwives was only one element. Efforts in health system strengthening in these countries have been characterised by: expansion of the network of health facilities with increased uptake of facility birthing, scaling up of the production of midwives, reduction of financial barriers, and late attention for improving the quality of care. Overmedicalisation and respectful woman-centred care have received little or no attention. |
