Résumé (français) : | Malgré les nombreux travaux sur la signification de la dépression post-natale peu de recherche a exploré la contribution d’abus passés ou présents sur celle-ci. La situation sur les abus et les symptomes de dépression ont été évalués chez 139 femmes au troisième trimestre de grossesse et à 2, 4, 6, et 8 mois en post-partum. Les prédicteurs de dépression post-natale ont aussi été évalués. Peu de femmes ont rapportés des abus actuels, bien que 37% aient rapporté avoir subit des abus physiques ou/et sexuels dans le passé. Les femmes ayant été victimes d’abus dans leur passé avaient 3.6-8.4 fois plus de risques de souffrir de dépression post-natale que les autres femmes, à chaque évaluation. Cet effet augmente régulièrement pendant les 6 premiers mois après la naissance et diminue au 8e mois. Ces résultats étendent les résultats de travaux récents sur la chronicité des abus et ont des implications pour les soignants. La prise en charge des femmes enceintes et en post-partum devrait prendre en considération les abus physiques ou/et sexuel passés. |
Abstract (English) : | Despite the growing body of evidence on the significance of postpartum depression, little research has explored the contribution of lifetime or current abuse to postpartum depression. One hundred-thirty-nine women were assessed during their third trimester of pregnancy and followed for 2, 4, 6, and 8 months postpartum for abuse status and depression symptoms. Predictors of postpartum depression were also assessed. Few women reported current abuse experiences, although 37% reported having lifetime physical or sexual abuse or both. Women with a lifetime history of abuse were 3.6-8.4 times more likely to experience postpartum depression than their nonabused peers at each measurement time. This effect steadily increases during the first 6 months after birth and decreases at the 8th month. These findings extend recent reports of the chronicity of abuse and have implications for care providers. The standard of care for abuse assessments may need to be expanded to include consideration of lifetime physical and sexual abuse experiences of pregnant and postpartum women. |
