Le National Birthday Trust a conduit une étude confidentielle sur les naissances à domicile au Royaume-Uni. Il y a eu un bon taux de retour de la part des sages-femmes, qui ont recruté prospectivement deux groupes de femmes : celles qui souhaitaient un accouchement à domicile et présentaient les conditions adéquates à 37 semaines, et un groupe similaire de femmes (« a matched group of similar women ») dont l’accouchement, à 37 semaines, était prévu à l’hôpital.

16% de ces femmes ont été orientées à l’hôpital en fin de grossesse (4%) ou en cours de travail (12%). Dans l’étude, ce chiffre monte à 40% pour les femmes primipares.

Le rapport présente une analyse de 4500 naissances à domicile et 3300 à l’hôpital (groupe de contrôle). Les résultats ont donc pu être présentés, soit en fonction de l’intention d’origine, soit de ce qui s’est effectivement passé. Il semble qu’une femme sélectionnée et suivie (screened) correctement en vue d’un accouchement à domicile n’a pas plus de risques pour elle et son bébé qu’une femme de profil similaire qui prévoit de donner naissance à l’hôpital.

Les naissances à domicile vont probablement atteindre 4 à 5% des grossesses au Royaume-Uni au cours des dix prochaines années, et cela doit être anticipé.

Abstract (English)  :

Obstetrics at Singleton Hospital, Swansea.

Recently the National Birthday Trust performed a confidential survey of home births in the United Kingdom. A good response rate was obtained from midwives, who recruited two groups of women prospectively; those planned and accepted as suitable for a home delivery at 37 weeks and a matched group of similar women who were booked for hospital by 37 weeks.

Some 16% of such women were transferred to hospital in late pregnancy (4%) or in labour (12%). This figure rose to 40% among the primiparous women in the survey.

The survey report presents an analysis of 4,500 home births and 3,300 hospital controls. Outcomes could therefore be presented by the woman’s intent or by what actually happened. In essence it seems that a woman who is appropriately selected and screened for a home birth is putting herself and her baby at no greater risk than a mother of a similar low-risk profile who is hospital booked and delivered.

Home births will probably increase to 4-5% of all maternities in UK during the next decade and this needs preparatory planning.
