Résumé (français) : | Cette étude descriptive a été menée pour évaluer le risque de rupture utérine en cas d’avortement tardif induit par voie vaginale chez les femmes ayant déjà subi une césarienne. 23 femmes ont été référées à un âge gestationnel moyen de 23,9 semaines (6,9) après une ou deux césariennes. Les indications d’interruption de grossesse étaient les maladies maternelles dans 4 cas et les anomalies fœtales dans 19 cas. Le RU 486 et les prostaglandines ont été utilisés pour la maturation cervicale et pour provoquer le travail. Une naissance vaginale a été obtenue dans 20 cas (86,9%) avec une durée moyenne de 72 (ET 52) h. La césarienne a été réalisée chez les 3 femmes restantes car aucune dilatation cervicale n’a pu être obtenue. Une rupture utérine s’est produite et a été traitée avec conservation de l’utérus. L’interruption tardive de la grossesse dans de tels cas peut être réalisée sans césarienne avec un taux de réussite élevé. |
Abstract (English) : | This descriptive study was conducted to evaluate the risk of uterine rupture in cases of late vaginal induced abortions among women with previous cesarean sections. 23 women were referred at a mean gestational age of 23.9 (SD 6.9) weeks of gestation after one or two cesarean sections. Indications for terminating the pregnancy were maternal diseases in 4 cases and fetal anomalies in 19 cases. RU 486 and/or prostaglandins were used for cervical ripening and to induce labor. Vaginal birth was obtained in 20 cases (86.9%) with an average duration of 72 (SD 52) h. Cesarean section was performed in the remaining 3 women because no cervical dilation could be obtained. One uterine rupture occurred and was treated with conservation of the uterus. Late termination of pregnancy in such cases can be achieved without cesarean section with a high success rate. |
