HISTORIQUE: La rupture d’un utérus non cicatrisé est rare, avec une incidence rapportée de 1 grossesse sur 8 000 à 15 000. Nous rapportons un cas survenu pendant le travail.

CAS : Une femme de 33 ans, gravida 3, para 0, avortement 2, a été admise à 40 semaines de gestation avec une rupture de la membrane. Une pression abdominale a été exercée pendant l’accouchement en raison de l’épuisement maternel La rupture utérine a été diagnostiquée par palpation des extrémités fœtales associée à une diminution du rythme cardiaque fœtal. Une lacération transversale de 6 cm a été découverte sur le segment utérin inférieur lors d’une césarienne en urgence. L’utérus était suturé. Il n’y a pas eu d’autres complications et le déroulement postopératoire s’est déroulé sans incident.

CONCLUSION : La rupture spontanée de l’utérus non cicatrisé pendant le travail est rare, un seul cas a été enregistré dans notre établissement sur une période de 10 ans. Les facteurs de risque incluent la faiblesse du muscle utérin et l’application d’une pression fondamentale. La détection précoce et l’intervention chirurgicale immédiate sont les piliers de la prise en charge.

Abstract (English)  :

BACKGROUND: Rupture of an unscarred uterus is rare, with a reported incidence of 1 in 8,000-15,000 pregnancies. We report a case occurring during labor.

CASE: A 33-year-old woman, gravida 3, para 0, abortion 2, was admitted at 40 weeks’ gestation with ruptured membranes. Fundal pressure was applied during delivery due to maternal exhaustion. Uterine rupture was diagnosed from palpation of the fetal extremities coupled with a decreased fetal heartbeat. A 6-cm transverse laceration was discovered over the lower uterine segment during emergency cesarean section. The uterus was sutured. There were no further complications, and the postoperative course was uneventful.

CONCLUSION: Spontaneous rupture of the unscarred uterus during labor is rare, with only one case recorded at our institution over a 10-year period. Risk factors include weakness of the uterine muscle and the application of fundal pressure. Early detection and immediate surgical intervention are the mainstays of management.
