Objectif
Cette étude visait à déterminer si le névrotisme et le niveau d’instruction prédisent le trouble de stress post-traumatique chez les femmes à la suite d’une grossesse infructueuse.

Méthode
Par le biais de publicités, les femmes enceintes dont la période de gestation était inférieure à 12 semaines ont été invitées à participer à une étude sur leur perception de la grossesse. Après avoir accepté, des questionnaires leur ont été envoyés, comprenant une échelle pour le névrotisme et leur niveau d’éducation atteint le plus élevé. Tous les deux mois pendant la grossesse et un mois après la date prévue de la naissance, on leur envoyait de brefs questionnaires sur la grossesse. Les participantes pour qui la grossesse s’était terminée sans succès ont été contactées par téléphone et invitées à participer à une étude de suivi avec une échelle de stress post-traumatique.

Résultats
Sur les 1339 femmes étudiées, 126 (9%) ont eu une grossesse infructueuse ; 8 d’entre eux ont abandonné l’étude (taux de réponse, 94%) ; 1 n’avait pas indiqué son niveau d’éducation. Les 117 femmes restantes ont rempli l’échelle de stress post-traumatique après environ un mois. Trente et une femmes (26%) répondaient aux critères du DSM-IV pour le SSPT et 86 femmes non. Une analyse de régression logistique a révélé que le SSPT était associé de manière significative à un névrotisme élevé, à un niveau d’instruction inférieur et à une durée de gestation plus longue.

Abstract (English)  :

Objective
This study aimed to determine whether neuroticism and educational level predict posttraumatic stress disorder (PTSD) in women following an unsuccessful pregnancy.

Method
Via advertisements, pregnant women with a gestational period shorter than 12 weeks were asked to participate in a study regarding their perception of pregnancy. After they had agreed, they were sent questionnaires, including a scale for neuroticism and their highest attained educational level. Every other month during the pregnancy and 1 month after the expected date of birth, they were sent brief questionnaires about the pregnancy. Participants for whom the pregnancy had ended unsuccessfully were contacted by phone and asked to participate in a follow-up study with a PTSD scale.

Results
Of the 1339 women studied, 126 (9%) experienced an unsuccessful pregnancy; 8 of these dropped out of the study (response rate, 94%); 1 had not indicated her educational level. The remaining 117 women filled out the PTSD scale after about 1 month. Thirty-one women (26%) met the DSM-IV criteria for PTSD and 86 women did not. Logistic regression analysis revealed that PTSD was significantly associated with higher neuroticism, lower educational level and longer duration of gestation.

Objetivo
Este estudo teve como objetivo determinar se o neuroticismo e o nível educacional predizem o transtorno de estresse pós-traumático (SPT) em mulheres após uma gravidez sem sucesso.

Método
Por meio de anúncios, solicitou-se que as gestantes com período gestacional inferior a 12 semanas participassem de um estudo sobre sua percepção da gravidez. Depois de terem concordado, foram enviados questionários, incluindo uma escala para neuroticismo e o nível educacional mais alto atingido. A cada dois meses durante a gravidez e 1 mês após a data prevista de nascimento, eles eram enviados breves questionários sobre a gravidez. As participantes para quem a gravidez terminou sem sucesso foram contatadas por telefone e solicitadas a participar de um estudo de acompanhamento com uma escala de SPT.

Resultados
Das 1339 mulheres estudadas, 126 (9%) tiveram uma gravidez sem sucesso; 8 deles abandonaram o estudo (taxa de resposta, 94%); 1 não havia indicado seu nível educacional. As 117 mulheres restantes preencheram a escala de SPT após cerca de 1 mês. Trinta e uma mulheres (26%) preencheram os critérios do DSM-IV para SPT e 86 mulheres não. A análise de regressão logística revelou que o SPT foi significativamente associado a maior neuroticismo, menor nível educacional e maior duração da gestação.

Le risque de développer un stress post-traumatique était négligeable chez les patients présentant un niveau d’éducation élevé et un faible score de névrose, alors que pour ceux dont le niveau d’instruction était faible et un score élevé pour le névrotisme, le risque estimé était d’environ 70%. Les soins et les conseils devraient être principalement axés sur ce dernier groupe.

For patients with a high educational level and low neuroticism score, the risk of developing Post-traumatic stress was negligible, while for those with a low educational level and a high score for neuroticism, the estimated risk was about 70%. Care and guidance should focus primarily on the latter group.

Para pacientes com alto nível educacional e baixo escore de neuroticismo, o risco de desenvolver estresse pós-traumático foi insignificante, enquanto para aqueles com baixo nível educacional e alto escore de neuroticismo, o risco estimado foi de cerca de 70%.
